Pathway Essentials: Statewide network, does not include UC Health, only available within the Denver metro area (10 counties around Denver) and Boulder.
Pathway Essentials Standard HMO: Network for Colorado Option plans purchased within the 10-county Denver Metro and Boulder areas.
Mountain Enhanced: Only available in these Colorado counties: Archuleta, Eagle, La Plata, Mesa, Moffat, Montezuma, Rio Blanco, Routt, and Summit.
Mountain Enhanced Standard HMO: Network for Colorado Option plans purchased within 1 of the 9 covered mountain counties.

Cigna Connect Network: Available in Denver, Boulder, Adams, Arapahoe, Broomfield, Douglas, El Paso, Jefferson, Larimer, Weld and Teller counties.
Cigna is leaving Colorado’s individual market for 2027. Their plans still work for the rest of 2026, so this directory is still the right one to check today - but a Cigna plan is not one to choose for next year.

Valley: Network along the Western Slope
Monument One: Mesa, Delta, Moffat and Rio Blanco Counties. Network for the CO Option plans on the Western Slope.
Monument Health: (More doctors) Mesa, Delta, Moffat and Rio Blanco Counties.
Choose Doctors or Locations, input name in "Keywords", Check if Dr accepts Commercial, CO Option, or KP Select under the Dr's name
KP Colorado (Commercial): Broadest network
KP Select: Includes all Kaiser providers and facilities with a
tailored network of affiliated providers and facilities.
KP Options: for Colorado Option plans.
*Best Practice: Call Dr's office and ask if they're in the "Select Health Value" network.
Select Health Value: In the purple box with the Network and City, state or zip - Make sure Network says Value and the area is correct. If you have to input your zip code make sure to select the city, state, and zip code from populated list, then click "Continue".
Select "Elevate Health Plans - Exchange/CO Option" or "Peak Exchange/CO Option", click "Continue". Search by name, specialty, or location.
Elevate: Aligned with Denver General Hospital, Denver Health
Winter Park clinic in Denver and Banner in the North.
Peak Plans: Aligned with Common Spirit, Summit Community Care
Clinic, Middle Park Health, Mercy Hospital (plus many more) in the
West.
